Self service password reset CAPTCHA limit reached

I have configured self service password resets in WAAD Premium but a user I'm testing this with consequently sees this message instead of the CAPTCHA image (translated from Dutch):The limit for the number of attempts has been reached. With these limits spam from automated programs is prevented. Please try again later.

However, this message was shown from the first time I ever visited the page so there have been no previous attempts. See also the screenshot below (in Dutch).
