Custom app integration- Password Based SSO

Hi,

I am trying to integrate my instance of BPPM (BMC Proactive Net) with Azure through Password Based SSO. When I try to access the app through the app panel, I am asked to enter my credentials, and then I get the error that 'The link is not working.' Any idea why Azure is not able to communicate with the app?
